Patient Records

• LAKESIDE CLIENTS: WE CANNOT PROVIDE COPIES (See Below).
• Please do not misplace your pets records! It is the responsibility of the pet parent to retain all records for their pet(s). 
• IMAGE COPIES OF VACCINE RECORDS $5.00 PER PET. 
• ELECTRONIC TRANSCRIBED RECORDS ARE $7.00 PER PET.
• PAYMENT: An email invoice will be sent to you (via email provided on this request form) for the records admin fee of $5.00-$7.00 PER PET.  Payment can be made by card.  
• Due to lack of resources we are only able to process your requests on Thursdays.   
• Please allow 3-5 days for your records copies. Due to lack of resources, We are not able process same day requests.

We will need to following information to process paper-records request: 

Name (Owner- as it was listed on your check in form & Pet), Location you typically visit and approximate last date seen. Paper records filed by date and locations.  We DO NOT have an electronic records system, ALL RECORDS ARE HAND PULLED. 

***LAKESIDE RECORDS: 

Lakeside is independently owned and operated. Records are solely managed by the lakeside team. Requests can be made in person at the Lakeside Location.  We DO NOT have records for Lakeside, WE CANNOT FORWARD REQUESTS.

This service only contains records from Nov 2024 to current, all other records from prior must be requested!
